Important: If you are unsure whether you already have an account or your previous
attempt was unsuccessful, please DO NOT create another account.

Account Creation Guidance
In order to create an account, you will be asked to complete the following steps:
Sign off on a Terms of Agreement
In accordance with New Jersey Department of Health (NJDOH) and OEMS administrative codes, authorized
staff (including NJDOH and approved system administrators/instructors) will access your record only
to verify certification, training, and legal background requirements for processing your credential(s).
Provide an Email Address
Use a personal email address that only you can access. Do not use shared or work addresses (e.g. staff@abc.com).
If needed, you can create a free email account with
Gmail,
Outlook, or
Yahoo.
Create a Password
Choose a password with at least 8 characters, ideally mixing letters, numbers, and special symbols.
Example: A-Z a-z 0-9 ! @ ^ * ( )
Provide a Social Security Number
Your SSN is required by N.J.S.A. 2A:56.44(e) to verify credentials and comply with state and federal laws.
It is used solely for credential verification by NJDOH. Misrepresentation may result in revocation of credentials.
Age Requirement
You must be at least 16 years of age to create an account.
